Employees of CHS Service Center – who work in Rockville Center, New York employees of CHS Service Center – who work in West Islip, New York employees of Nursing Sisters Home Care who work in or are supervised from Westbury, New York immediate family members or households of the groups listed above.
Household is defined as persons living in the same residence maintaining a single economic unit.
